The two clips clip to the outer labias as shown in the picture. If they are too tight, they can gently be bent outwards. But they should be just right as they are.
A natural and freely spinning gemstone (pick your favorite one!) will then sit right on your pearl and get things going, while the two chains will daintily dangle when in motion. :)
