Most adults are comfortable between 1.5 and 1.75 inches (38 to 44 mm). If you have a small mouth, jaw issues, or want to wear it for longer than a few minutes, start at 1.25 inches or below. Go up a size only once the current one feels easy.

Open your mouth to a comfortable width, not your widest, and measure the gap between your top and bottom front teeth. Choose a ball a little smaller than that gap: the strap will pull it in, and a ball that is too big forces the jaw open and gets painful fast.

Silicone is body-safe, easy to wash, and slightly soft on the teeth, which is why it is the default for anything you wear for a while. Leather and rubber balls look the part but need more care and can taste of the material.

A ball gag muffles and fills the mouth. A ring gag holds it open instead. A panel gag covers the mouth with a leather panel and is the quietest of the three. A harness adds straps over the head so the gag cannot be pushed out and doubles as restraint. Bit gags sit between the teeth and are mostly a pony-play thing.

With rules, yes. Agree a non-verbal signal before the gag goes in, never leave a gagged person alone, avoid anything with a stuffy nose or after drinking, and keep sessions short at first. Breathable designs (holes in the ball, ring gags) are the safer choice for longer wear.
